Medicare Just Got More Complex for 2027 Enrollment

Fewer plan choices open Oct. 15, and Humana’s star surge is moving stocks.

Thursday, October 15, Medicare’s annual open enrollment window opens. For the roughly 69 million Americans on Medicare, it arrives with a quiet but consequential change: fewer plans to choose from, and in many counties, meaningfully higher costs to stay covered.

The Shrinking Menu

For 2027, the typical Medicare beneficiary can choose from 28 Medicare Advantage prescription drug plans, a drop from the 32 available in 2026, according to KFF. Count all Advantage plans together and the average beneficiary can choose from 35 plans, compared with 39 in 2026. That sounds modest until you consider who is losing choices and why.

Millions of enrollees will be forced to find new coverage for 2027 as insurers pull back from markets to protect profit margins rather than chase subscriber growth. There will be no Medicare Advantage plans in 181 counties in 2027, up from 67 in 2026. The retreat is not random. UnitedHealth Group is exiting 228 counties and entering just 9 new ones, while Humana is leaving 62 counties and entering 4, according to KFF.

For anyone whose plan is discontinued, the practical advice is simple: beneficiaries whose plans are being discontinued should receive non-renewal notices from their insurers and can compare 2027 options using the Medicare Plan Finder at Medicare.gov or by calling 1-800-MEDICARE. The shrinking menu matters most for people whose current plan is leaving their county or changing its doctors and drug list, since fewer options can mean fewer networks to choose from, which raises the stakes for anyone managing a chronic condition or taking several prescriptions.

The Investment Case on the Other Side

What is bad news for beneficiaries hunting for a plan is, in a straightforward way, good news for the insurers that chose where to stay. Retreating from unprofitable counties is not a sign of weakness, it is a deliberate shift in risk and cost exposure. UnitedHealth Group, Humana, and CVS Health subsidiary Aetna are among those exiting markets or tweaking their coverage offerings as they try to protect profit margins instead of pushing for growth in new subscribers, analysts said.

The star-rating data released by CMS last Thursday reshuffled the competitive order in a way that matters for investors. Humana, which suffered a major ratings collapse two years ago, saw the percentage of its members in 4-plus star plans rocket from 41% in 2026 to 93% in 2027, according to a Healthcare Dive analysis of CMS data. Higher ratings lead to bonus payments from the government and can boost plan enrollment. The market priced that in fast: Humana’s shares rose about 15% on Friday after analysts named it the top beneficiary of the new star ratings for 2027 Medicare Advantage plans.

UnitedHealth’s position is more complicated. J.P. Morgan estimates UnitedHealth’s share of enrollments in plans rated four stars or higher will fall to about 67% from 81%, and that of CVS Health to roughly 70% from 84%. The step-downs for UnitedHealth and CVS are manageable, especially because their enrollment in 4-plus star plans is still at the higher end compared to their peers. But the direction of travel is worth watching heading into earnings.

Building Wealth Around This Idea

The long-term structural case for Medicare Advantage remains intact. More than half of eligible Medicare beneficiaries are enrolled in Medicare Advantage in 2026, according to KFF. An aging U.S. population means that market grows each year regardless of which insurer holds the county.

Humana is the clearest near-term catalyst given its star-rating rebound, but the stock moved about 15% in a single session, so position sizing matters. Investors adding here are paying for a recovery that is already partially reflected in the price. UnitedHealth, whose county exits are among the most aggressive of any major insurer, is deliberately trading scale for margin, a move that tends to reward shareholders over 12 to 24 months once medical-loss ratios stabilize.

Risks to Monitor

CMS controls both the reimbursement rates and the star-rating methodology. Any unfavorable rate revision in the 2028 cycle could quickly reverse margin gains. The average number of Medicare Advantage prescription drug plans available to a beneficiary has dropped from a peak of 36 in 2024 to 28 for 2027, according to KFF, a contraction that is drawing scrutiny. Regulatory pressure on insurer exits is a real risk, particularly ahead of a political cycle where healthcare costs poll badly.

Daily Wealth Takeaway

When a large market gets less competitive, the remaining players with the strongest quality scores and the most disciplined cost controls are positioned to capture durable pricing power. That is the story playing out in Medicare Advantage right now. For readers who are also enrollees, use this window to compare plans at Medicare.gov before December 7. For investors, the opportunity is in identifying which insurer’s discipline today becomes next year’s margin.
